The Gift Card is a more personal and secure gift than cash. You choose the amount you want to place on the card, anything from as little as $25 up to as much as $5,000, and the recipient chooses the health related expenses he or she wants to use it for — however there’s no rule that says you can’t make suggestions!

The card costs $4.95 plus shipping and handling and can be sent to you or straight to the recipient.

Uses include: co-pays at doctor’s and pharmacies; dental care including braces and teeth whitening; vision care including eye exams and prescription sunglasses; memberships at health clubs and elective procedures including Lasik and cosmetic surgery. Receiving the Gift Card

There are a couple of important points for you as the Gift Card Giver, to keep in mind for the Gift Card Receiver. The Prepaid Healthcare Visa Card should be used at merchants and service providers that accept Visa Debit Cards.

Also, the gift card cannot be used at any ATM and can not be used in exchange for cash. The card officially becomes the property of the recipient as soon as they sign the back . Simple activation instructions will be included with the gift card.

Card Maintenance

Beginning in the ninth month after the Gift Card is purchased; there is a monthly maintenance fee of $1.50 per month, as long as the card retains funds.

If a personalized Healthcare Gift Card is lost or stolen, the recipient must contact 1-877-850-3774 and a customer service agent will assist them in replacing the card and protecting their card balance. There is a fee of $4.95 to replace a lost or stolen card that will be reduce from the balance of the card.
