What is the minimum amount you can put on the prepaid healthcare Card? The maximum?

$25 is the minimum amount; $5,000 is the maximum.

Beginning the ninth month after the card is purchased, there's a monthly maintenance fee of $1.50.

Why does the Healthcare Gift Card say "Debit" on the front of the Card? Do I need a PIN Number?

The Healthcare Gift Card should be used just like a Visa credit card. There is no need to enter a PIN number. When making purchases at the checkout, doctor’s office, online or for any other health care expense, use the card as you would a credit card or by selecting “Credit” at the cashier. You will be allowed to use the card as many times as you want until the balance is used up.
